We have tried to make this website useful to you in few different ways.

  • Become a Bayside patient. The easy steps are outlined.
  • Prepare for your visit material. Forms and handouts to download at home prior to a visit.
  • Health information links and handouts in our Health Library.
  • Find us. Our local office locations, hours, and clinicians.
  • Contact our administration or our business office, by telephone, fax, or email. Contact us.
  • Contact us out of channels. For circumstances beyond the ordinary, when something isn’t working right, when going through normal channels has failed, if frustration has set in (we hope this is rare), email our Patient Advocate at advocate@baysidemedical.com.
